2024 Health Exam Form
The Health Exam Form is emailed to you when we confirm your registration in summer camp. You need to print this document and have a licensed healthcare provider complete and sign it in order for your camper to attend camp.  All campers must have had a health exam by a licensed healthcare provider within the 12 months preceding the start date of the camp session in which they are registered. Please bring the completed form with you on check-in day.
